Sharon Springs Dental Friends and Family Plan
With your Friends and Family Plan, there are:
- No yearly maximums
- No deductibles
- Immediate eligibility (no waiting periods)
- No claim forms
- No pre-authorization requirements
- No pre-existing condition exclusions
Terms and Conditions: The Wellness Plan is not insurance. All plan membership fees are non-refundable and are due at the time of registration. Membership will automatically renew on the anniversary date unless a written cancellation request is received 30 days before the renewal. The plan duration is 1 year from the date of enrollment. The Wellness Plan is stand-alone coverage not to be combined with insurance benefits.
